What you will need

Before proceeding with your application please ensure you have the following available to you:

• A scan or clearly readable photograph of your registration certificate (eg. Ofsted). If you provide care at a school you will need a copy of documentation containing the school’s registration number, such as a page of an inspection report.

  You will be prompted to upload a copy of your certificate or equivalent documentation at the end of this application. Please prepare a scan or a full size, clearly readable photograph.

  You may provide a fax or posted photocopy if you are not able to provide a scan or photo.

• Your payment details. If you wish to be paid directly into your bank or building society account, the following will be required:

  • Account Holder
  • Bank or Building Society Name
  • Sort Code
  • Account Number
  • Roll Number (if applicable)
